Home and community support services are designed to help individuals with disabilities, complex health conditions, or aging-related needs maintain their independence and quality of life — in their own homes and as active members of their communities.
At SFCS, we know that the right support looks different for every person. Some individuals need a few hours of assistance each week with household tasks or personal care. Others need regular accompaniment in the community for social activities, appointments, or skill development. We work to understand what each individual needs and match them with support workers whose skills and approach are the right fit.
Our community support workers are experienced, patient, and genuinely invested in helping the individuals they support to thrive. All are screened, credential-verified, and have passed vulnerable sector background checks before joining our network.
Types of Support Available
Our support workers can assist with a broad range of needs — tailored to the goals and preferences of each individual.
In-Home Personal Care
Assistance with daily personal care activities including bathing, grooming, dressing, oral hygiene, and toileting — delivered with dignity and respect for individual preferences.
Household Assistance
Light housekeeping, laundry, meal preparation, grocery assistance, and help with household organization to support a safe, comfortable home environment.
Community Access
Accompaniment to community activities, recreational programs, social outings, libraries, faith communities, and other locations the individual chooses to be part of.
Transportation & Appointments
Accompaniment to medical appointments, therapy sessions, day programs, and other important commitments — ensuring the individual arrives safely and is supported throughout.
Skill Development
Support with building practical life skills — cooking, budgeting, using public transit, job readiness, and other areas that increase independence and self-confidence.
Social & Recreational Support
Encouraging and facilitating participation in hobbies, sports, arts, community events, and social relationships that contribute to joy, belonging, and overall well-being.
Who This Service Is For
Home and community support services from SFCS are available to individuals across Ontario who require assistance with daily activities, community participation, or independent living — including:
- Individuals with intellectual or developmental disabilities
- Adults and youth with physical disabilities or chronic health conditions
- Individuals with acquired brain injuries
- Seniors requiring assistance to remain living at home
- Individuals transitioning from institutional care to community living
- Individuals with autism spectrum disorder
- Anyone with a documented need for daily living support

Contact us by phone or through our online care request form. Tell us about the individual's situation, needs, and what kind of support you're looking for.
Comprehensive Intake Conversation
We'll have an in-depth conversation with the individual and/or their family to understand goals, preferences, daily routines, communication needs, and any specific support requirements.
Support Worker Matching
We identify support workers from our network who have the right skills, experience, and personal qualities to be a good fit for the individual's needs and personality.
Introduction & Confirmation
We facilitate an introduction meeting before regular support begins. The individual and family have the opportunity to meet the proposed worker and confirm the match is right.
Support Begins & Evolves
Regular support begins on the agreed schedule. We check in periodically to ensure things are going well and adjust the plan as needs or goals change over time.
